Revision Nose Surgery

Secondary Rhinoplasty – Revision Nose Surgery

Rhinoplasty is the most common plastic surgery in Turkey: it is a procedure that men and women perform for functional reasons (such as difficulty breathing) and for aesthetic reasons only.

However, the trend that has been confirmed in recent years is the second rhinoplasty, which is called secondary rhinoplasty, that is, it corrects mistakes or embarrassments that occur after the first nose surgery or primary rhinoplasty.

The nose is located in the middle of the face, so it is not difficult to understand that its position makes it one of the facial details on which the gaze is most focused. The nose cannot be ignored in assessing the overall harmony of the face. When asked which part of the body you would like to change, it is no coincidence that 80% of the answers are only about the nose.

For these reasons, when you decide to change the shape of your nose, you should consult a specialist to avoid having to correct it as much as possible in a secondary operation.

Secondary Rhinoplasty

Among the new trends in rhinoplasty is the second touch with a corrective function of what happened in the initial surgery. More and more men and women are resorting to secondary rhinoplasty if the results of the first rhinoplasty are unsatisfactory or if there is a change in aesthetic expectations.

A second nose surgery may be required for aesthetic reasons, breathing problems or a trauma-related defect. The first surgery may not fix all existing problems (eg due to trauma). However, since we are not satisfied with our first rhinoplasty, we like to apply for a new intervention more often.

However, it must be said that secondary rhinoplasty is a more delicate operation than the previous one, because it completely overlaps the previously operated area. In any case, the appropriate time (usually 8 months) must pass for a good evaluation of the results of the first surgery in order for the decision to intervene a second time to be made completely.

What is Revision Nose Surgery

Revision rhinoplasty (secondary rhinoplasty) is a cosmetic surgery that must be repeated after one or more operations of the same type whose results are unsatisfactory for the patient.

There are different types of secondary rhinoplasty depending on the defect to be corrected, although they are not always simple procedures. The result largely depends on the initial condition and the clarity of the clinical picture to be treated.

Why is Revision Nose Surgery Performed?

Statistics tell us that rhinoplasty is one of the first surgeries requested by both women and men, and ironically, this constant increase in requests is bringing with it more and more dissatisfied patients. In most cases, a second intervention is required to perform the minimal form of cosmetic corrections, and in other cases it is necessary to intervene to correct the previous one: for example, it is an operation to reshape the nasal septum, which completely ends the main defect. If the bone and cartilage are not properly intervened during the first rhinoplasty and the result is not satisfactory, then the defects are corrected through secondary rhinoplasty.

The bridge of the nose and the tip of the nose are the most common cosmetic corrections in first rhinoplasty. However, an intervention on a nose that has been operated on before is a very delicate procedure that should be carefully evaluated by a competent surgeon. As in the case of primary rhinoplasty, it is necessary to contact a surgeon for a review or for a secondary rhinoplasty. Not all plastic surgeons specialize in facial surgery, so you need to choose wisely and learn to choose your surgeon before undergoing a second surgery.

The goal of secondary rhinoplasty

The main purpose of secondary rhinoplasty is to correct the result of the previous operation that did not meet the patient’s expectations. There may also be a repetition of this experience at the same surgeon, because even a doctor with long experience may encounter a completely incorrect result.

Symmetry is essential in this area and secondary rhinoplasty can also be defined as “millimeter surgery”.

Therefore, it is important to trust a specialist who has proven experience and may find himself having to repeat the surgery.

To achieve this accuracy requires a thorough knowledge of the anatomy of the nose, an indispensable condition for obtaining the best possible result.

What can secondary rhinoplasty (secondary rhinoplasty) fix?

For primary rhinoplasty, the professionalism of the surgeon is more certain than for secondary rhinoplasty. A second nose job should not be considered unless you are absolutely convinced that it is really necessary. The specialist must first assess how the patient’s nose has responded to previous surgeries, and then anticipate the new scars that will occur after the second surgery. However at this stage, it will be possible to plan the most effective surgery according to the expected and realistic result that is achievable.

The demand for both primary and secondary rhinoplasty is increasing towards reducing the size of very large noses or concealing defects that are still very visible.

The Importance of Computer Simulation of a Perfect Nose

Computer simulations help plastic surgeons give the patient a clear idea of what the outcome of a secondary rhinoplasty will be.

Thanks to special programs, the patient can see the effect of the desired pattern on his face in order to understand what result will be achieved and, if necessary, correct it to obtain a more natural and more harmonious effect with all other elements. In this way, we try to eliminate the possibility of feeling the need for a “third” touch after revision rhinoplasty.

Failed Rhinoplasty: Reasons

A failed primary rhinoplasty can have various causes, such as tissue failure that does not show the expected reaction during the recovery phase, or complications such as swelling, edema or infection after surgery.

Minor defects can often be treated in the clinic, for example; With a corrective application that involves the tip of the nose and requires only local anesthesia.

It is a virtually painless procedure and is performed in a short time.

Defects that need to be corrected by secondary rhinoplasty

The plastic surgeon decides to perform a secondary rhinoplasty when the damage caused by the primary surgery is clearly negative.

distorted and asymmetric nose shapes; It is a bad result and has negative repercussions on the patient’s self-esteem and deterioration of his life relationship.

The face is the first business card in any personal relationship, and aesthetically pleasing is now in demand.

For this reason, the demands for secondary rhinoplasty for further correction and sometimes because the expectations of the nose previously imagined before the primary surgery are not met.

The most common defects after a failed rhinoplasty are:

  • Twisted, bent and mismatched tip;

  • Defects resulting from the removal of excess cartilage and bone tissue.

  • Defects in the columella; Any part that divides the nostrils.

  • Asymmetrical nostrils

  • Deviation of the nasal septum

It is not uncommon for more than one of these defects to occur together, because some of them have the same causes.

Secondary rhinoplasty is usually done with a small incision in the nasal stent just below the tip of the nose to make the surgical scars completely invisible.

This technique is generally known as “opening”.

However, it is necessary to know that a nose that has undergone surgery or different interventions has criticisms in the same surgical intervention and in terms of the result that the patient desires. However, a good surgeon can still achieve the optimum aesthetic level.

When can secondary rhinoplasty be performed?

Revision rhinoplasty (secondary rhinoplasty) can only be performed after at least 10 months after the previous surgery due to waiting for the remodeling and stability of the cartilaginous tissue and its stability, which in most cases needs retouching.

in this meaning; It may be necessary to take autologous material (from the same patient) from a portion of the cartilage or bone of the patient himself. This eliminates the risk of a potential postoperative rejection reaction.
